Nursing Process
A systematic and rational method of planning and providing patient care organized around a series of phases that facilitates evidence-informed and ethical nursing practice.
Assessment
The step in the nursing process where the nurse gathers and analyzes information to understand the client’s health status.
Nursing Diagnosis
A clinical judgment about individual, family, or community responses to actual and potential health problems which directs the plan of care.
Planning
The phase of the nursing process where the nurse prioritizes proposed strategies and interventions and creates a client-centered care plan.
Implementation
The process of carrying out the nursing care plan, involving both nurse-initiated and physician-initiated treatments.
Evaluation
The step where the nurse assesses the achievement of outcomes to determine if the interventions have been effective.
Diagnostic Reasoning
The process of using assessment data about a client to logically explain a clinical judgment.
Collaborative Problems
Actual or potential physiological complications that nurses monitor to detect changes in a client’s status.
NCSBN Clinical Judgment Measurement Model (NCJMM)
A framework which outlines the clinical judgment process by recognizing cues, analyzing cues, prioritizing hypotheses, generating solutions, taking action, and evaluating outcomes.
NANDA Nursing Diagnosis
A taxonomy of nursing diagnoses providing a standardized language and framework for describing clinical judgments.
