1) Get complete history of dog

  • Did it get into anything, urinating/defecating normal, vaccinated, other pets, travel recently?

2) Is it functional or painful walking?

  • Dog is seen walking oddly, definitely a neurologic problem, but could be musculoskeletal too so could be painful or nerve related

Dog is walking with tail up, happy, so NOT painful but functionally inhibited

3) Is every limb affected, is the head affected?

  • is it a generalized lesion affecting the whole dog?

4) If not painful, then what is it?

  • Generalized weakness

    • Weakness is a huge red flag for being a neurologic problem; should always be considered (unless Addison’s (endocrine, rarer))

Dog given drug; walks fine almost immediately after - what?!

Potentially:

1) Junction between motor and nerve muscle not working correctly

2) Coonhound paralysis

  • Motor nerve itself stopped functioning

3) Myatonia

  • Muscles contract too much because they have ion problems

4) Myasthenia gravis

  • Very rare

  • Antibodies are made that target NMJ specific proteins

  • May kill proteins entirely or make it so they don’t work anymore

The drug given was Tensilon

  • Takes remaining proteins left in the cleft and give them more time to respond

  • Not curing, but delivering way more by inhibiting the enzyme that breaks it down

LOCALIZE THE LESION

  • In this case, its diffuse - a generalized lesion

    • In ALL NMJ around the body

  • Will try to suppress the immune system with a steroid to manage this

  • Synapse-related case
