2. Describe the 5 steps of the Pharmacists’ Patient Care Process.
Collect: Pharmacist collects relevant and necessary subjective/objective information about the patient and is responsible for analyzing data in order to understand the relevant medical/medical history and clinical status of the patient. (med list, prescriptions/non-prescriptions/herbals, med history/biometric tests, patient lifestyle)
Assess: Assess information collected and form problem list of patients active medical problems and medical therapy to prioritize recommendations to improve patient health. (medication safety/effectiveness, health/functional status, immunization status)
Plan: develop a patient center plan. address meds, identify goals, updated med list, personalized education for patient, support care continuity
Implement: implements care plan in collaboration with other health care professionals. discuss plan, provides med list, schedules follow up, documents encounter
Follow up/monitor and evaluate: Provides ongoing follow up to make sure meds are doing their job. Outcomes of care.
