Separate baseline, treatment, probe data points/Label x-axis, y-axis, condition labels, data points, data path/Include caption/Label x-axis in equal intervals/Indicate breaks in treatment on x-axis/Scale y-axis to represent true values/start the y-axis at 0

Treatment Summaries
Serves as a progress report that summarizes effects of treatment over a given time period/look very different + may be provided at different times depending on setting
Identifying Information
Provide same info included in treatment plan/discuss specifics regarding treatment schedule/ Period covered; From to/Sessions per week/Length of sessions/Number of clinic sessions/Total clock hours of therapy
Background Information
Provide same info included in the treatment plan/dates for which treatment was provided/overall length of time client has participated in therapy
Objectives and Treatment
Long-term goal #1: Copy from treatment plan/Objective #1: Copy from treatment plan
Objectives and Treatment Procedures
Summarize from treatment plan/Include any modifications to the treatment plan were made
Objectives and Treatment Progress
Begin with a simple statement about whether or not the objective was met/Give baseline data, analyze progress over treatment course, and give end-of-the-semester probe results
Summary and Recommendations
Summarize client's progress/If appropriate; describe subjective impressions on how semester went/Make recommendations regarding if treatment should continue/If recommending further treatment&appropriate, include suggestions for future target behaviors
