Care Objective for Tachycardia

Rapid termination of life threatening arrhythmias and transport to a facility capable of definitive care.

Care Objective for Tachycardia

Rapid transport to facilitate the treatment of the arrhythmia where treatment is not available in the prehospital environment.

Ventricular Tachycardia requiring management

Lasting > 30 seconds,

Rate > 100,

QRS > 0.12 seconds,

Regular (mostly) AV dissociation / absence of P waves

Signs & symptoms of unstable / rapidly deteriorating patient

Inadequate perfusion / shock (e.g. hypotension, pallor and diaphoresis),

Acutely altered conscious state / LOC,

Ischaemic chest pain,

APO

Stable: VT / unclear

Amiodarone infusion 5mg/kg (max 300mg) over 20/60min

Dilute with Dextrose 5%

Unstable: VT / rapidly deteriorating

Syncronised cardioversion

Midazolam 1-2mg & Fentanyl 50mcg IV (as needed)

Cardioversion: DCCS 150j - repeat once

If unsuccessful pads ANT-POST and DCCS 200j.

No reversion or reversion to narrow complex

Amiodarone infusion 5mg/kg (max 300mg) over 20mins
