Common causes of DIC
Cancer, tissue injury, burns, Infections(sepsis), toxins, pregnancy, pancreatitis, liver disease
How do you treat stage 1 of DIC?
Use low dose heparin or low dose low molecular weight heparin»to control coagulation
How do you treat stage 2 of DIC?
Replace missing cofactors (AT3) with plasma; then after repletion, Factor V and VIII must be anticoagulated
How do you treat stage 3 of DIC?
Use amicaproic acid (antifibrinolytic) to inhibit plasmin, use heparin to inhibit thrombin, use plasma+cryoprecipate to replenish coagulation factors
What is stage 1 of DIC?
Stage 1: High TF release, AT3 is adequate to control cascade BUT Factor V and VIII are being activated by thrombin>>more coagulation>>Risk of thrombosis
What is stage 2 of DIC?
Stage 2: AT3 gets consumed, excess thrombin is produced>>Protein C gets active and aPC inactivates Factor V and VIII resulting in their deficiencies>>activated platelets are removed and risk of bleeding is increased
What is stage 3 of DIC?
Stage 3: aPC inhibitor gets consumed, fibrinolysis activated>>free plasmin dissolves clots throughout the body leading to rebleeding of old wounds
