bladder retention
inability to empty the bladder
over distended bladder
This causes the fundus to displace
over distention causes the
detrusor muscle to not be able to contract
hydronephrosis
abnormal condition of water in the kidney
kidney becomes stretched and swollen due to build up of urine
vesicoureteral reflux (VUR)
disorder caused by the failure of urine to pass through the ureters to the bladder, usually due to impairment of the valve between the ureter and bladder or obstruction in the ureter
Urine move back to kidneys
mechanical obstruction of bladder
something is physically blocking passage for urine
renal calculi
kidney stones
who is at highest risk for bph
old men
risk factors for urinary retention
old age
men
diabetes
constipation
history of uti
immobility
medication
why does constipation cause urinary retention
the fecal matter puts pressure on bladder
S/S of acute retention
occurs suddenly and painful inability to void despite full bladder
possible EXTREME PAIN
severe bladder distention
Medical Emergency
Bloating
Caused by: medications, surgical procedures, Uti, excessive fluid intake, BPH
s/s of chronic retention
PAINLESS: associated with increase residual urine volume
difficulty starting and maintaining urination
weak flow
can lead to overflow incontinence
Bladder scan to find residual urine volume
is done to determine if the client emptied the bladder completely.
chronic retention range
25ml to 50ml urine at a time in frequent intervals
Firm distended bladder, may be
displaced to one side of midline.
(may cause urinary retention)
percussion will have a
dull tone over bladder
if residual urine is greater then 100mL then the
individual has urinary retention
How to promote urinary elimination
encourage normal voiding pattern
apply warmth to relax sphincter
warm caffeine free drinks
cholinergic medications help
stimulates the bladder to contract more (detrusor muscle)
1 ounce equals
30 mL
2 tablespoons
Pathophysiology of Urinary Retention
Bladder emptying is impaired
Urine accumulates
Bladder becomes overdistended
Overdistention causes poor contractility of detrusor muscle
Furter Impairment of Urination
