Before signing a consent form for surgery the patient says I'm not sure about the possible risk. What should the nurse do first?
Inform the primary care provider that the patient has concerns about surgery
If a patient says in the preop, I have changed my mind. I don't want to go through with surgery. What should the nurse do first?
Notify the healthcare provider of the patients decisions. We don't ever want to try to convince them or suggest that they should talk over the decision with their family members. The primary care provider should be notified before the surgery department.
What things would we remove to decrease a fire hazard?
A scented candle, a portable space heater under the bedside table, tile draped over a lamp for dim lighting, a power strip attached to hospital bed. We would not have to remove alcohol base hand sanitizer on the wall or unused oxygen tanks.
A chart is observing an LPN preparing to irrigate a patient's dwelling urinary catheter. The nurse would intervene when the LPN initiates what action
Opens bottle of sterile distilled water to flush. We only use sterile normal saline to flush catheters.
How many times should older adults shower per week?
3 to 4 times. So if a patient is bathing six times per week that is something that we would want to teach them about. We also want them putting on moisturizer because their skin gets more dry.
How should we teach a patient to move with a cane?
If the patient has a right sided weakness, we would tell them to hold the cane in their left hand, move the cane, and the right leg forward together, maintain the cane at the height of a greater trochanter. Keep the elbow bended, and when they go up the stairs we want them to step with their strong leg so it would be the left leg first and down with their weaker side so the right leg
what things can a nurse in an OR do to prevent surgical site infections
Positive pressure can be used in rooms to ensure that pathogens do not contaminate the patient or supplies. They are typically considered the cleanest rooms in the hospital, keeping the doors open and Pete the air exchange in the OR. The air exchange system is designed to decrease airborne contaminants in the OR limiting the traffic in the room decreases the amount of bacterial shedding, minimize harmful air and prevents accidental contamination of the sterile field.
PUSHES THINGS OUT
what would we use negative airflow for
Negative airflow refers to an airborne infection isolation room negative pressure room in a hospital is used to contain airborne contaminants within the room things like bacteria virus, fungi, molds gases chemicals. For example, example patients with active TB, you can better contain the bacteria within the room
Keeps thing IN
the nurse is preparing a patient to be transported to get a left lung tissue biopsy. What should the nurse confirm has been completed
Consent form pre-op medications are giving most recent lab work is on the chart and the person performing the procedure. Has the site marked. We would prep the site with clippers not a razor. The goal of pre-op skin preparation is to decrease bacteria without injuring the skin
how much should the nurse assist a postop patient transferring from the bed to a chair?
Ensure that the patient's feet are as wide as the hips. Do not teach them to look down and wash their feet as they move or bend at the waist or place a walker away from the bed.

Determine if this is appropriate or inappropriate

A nurse is assessing sensory deficits. What assessment should the nurse include?
Asking about changes in vision, observing conversations with others, assessing two point discrimination, which is based off of feeling, and performing the rinne test for hearing
for near vision we use the Rosenbaum eye chart, not the snellen eye chart
While performing wound care to a donor skin graft site, the nurse notes some scabbing around the edges and a dark collection of blood. What is the nurse’s next action?
3. Gently remove the debris and re-dress the wound.
We will not leave the scabbing area alone and apply extra ointment because bacteria likes to live in the scabs and dry blood so we want to remove the debris to prevent infection
How do you calculate total burn surface area using the rule of nines

Posterior trunk = 18
Posterior arm = 4.5
TBSA burned = 22.5%
