What is the drug class of GTN?
Vasodilator.
What is the pharmacology/MOA of GTN?
Vasodilates veins/coronary arteries → ↓ preload, ↓ BP, ↓ cardiac workload.
What is the main effect of GTN?
Decreases preload via venous dilation.
How is GTN metabolized?
Absorbed rapidly, metabolised by liver.
What are the indications for GTN?
Suspected ACS (with pain), APO, autonomic dysreflexia, irukandji syndrome.
What is the BP requirement for autonomic dysreflexia/Irukandji?
SBP ≥160 mmHg.
What is contraindications of GTN (ASHCIR)
Allergy/ADR, SBB <100 mmHg, HR <50/>150 bpm, CVA, head trauma, PDE5 inhibitors within the last 24 to 48hrs, riociguat medication
What is a contraindication for GTN related to heart rate?
HR <50 or >150 bpm.
What is a contraindication for GTN related to blood pressure?
SBP <100 mmHg.
What is a contraindication for GTN related to neuro conditions?
Acute CVA/head trauma.
What is a contraindication for GTN when using sildenafil/vardenafil?
Previous 24 hrs.
What is a contraindication for GTN when using tadalafil?
Previous 48 hrs.
What is a contraindication for GTN related to other medications?
Riociguat.
What is a precaution for GTN
Inferior AMI, Cerebral vascular disease, Hypotension/syncope, Effects enhanced with intoxication, Caution if taken within previous 4 days
What are common side effects of GTN? DHS-RV
Dizziness, hypotension, syncope, reflex tachycardia, vascular headaches
Adult does
400 mcg Subling, rep 5 mins no max dose
What is the onset time for GTN?
<2 minutes.
What is the duration of GTN's effect?
20-30 minutes.
What is the half-life of GTN?
5.5 minutes.
What is the recommended patient position for GTN administration?
Sitting or semi-reclined.
