When Vaccines Should Not Be Given

Live vaccines to immunocompromised children; prior allergic reaction (anaphylaxis/generalized rash); seizures; fever >101°F.

Normal Vaccine Reactions

Low-grade fever

Immunocompromised Patients

IVIG administration; steroids >2 weeks; pregnant women; neutropenia; chemotherapy.

Vaccines During Severe Immune Compromise

Okay: DTaP, Hib, Hep A/B, HPV, meningococcal, PCV, IPV, Tdap. No: influenza, MMR, rotavirus, varicella.

General Vaccine Allergy/Precautions

For ALL: prior-dose anaphylaxis, current high febrile illness, severe latex allergy.

DTaP Considerations

Delay with unclarified neurologic diagnosis or history of Guillain-Barré.

Hepatitis A & B Considerations

Hep A: test first if severe allergy to alum/neomycin. Hep B: test first if severe allergy to yeast.

HPV Considerations

Do not give in pregnancy.

Influenza Considerations

Do not administer during severe immune compromise; test first if severe egg allergy.

Meningococcal Considerations

Do not give with history of Guillain-Barré.

MMR Considerations

Do not administer during severe immune compromise; delay during pregnancy or if attempting pregnancy within 1 month; test first for severe gelatin, egg, or neomycin allergy.

Polio (IPV) Considerations

Do not give with severe allergy to -mycins.

Rotavirus Considerations

Do not administer during severe immune compromise; precautions with severe GI conditions.

Varicella Considerations

Do not administer during severe immune compromise or pregnancy; test first for severe gelatin/neomycin allergy.

Anticipatory Guidance

Proactive approach focused on the child's needs at each stage; education provided by RN/HCP; promotes safety.

Pediatric Measurements

Head circumference; height vs. length; weight.

Infant Screenings

Birth: mandated newborn screen. 9-12 months: iron-deficiency anemia + lead screening. 12 months: developmental screening.

Toddler/Preschool Screenings

Autism: 18 + 24 months; vision: 3-5 years; hearing: 4 years.

School-Age/Adolescent Screenings

Scoliosis: 10-16 years; depression/anxiety (PHQ-9): 12 years; STI/HIV: 15 years.
