Long tem care
For those who require 24 hour skilled care (medically necessary care)
AKA SNF, Rehab center or extended care facilities
Long of stay may vary
Home Health Care
At the person’s home
usually for older or chronically ill patients
Assisted living
For residents who need some help with daily tasks but can live in a more independent environment
Adult Day Services
for people who need help or supervision during certain hours (may also give a break to family and friends)
Acute Care
24 hour skilled care given in hospitals or ambulatory surgical centers
for immediate short term care for illness or injury
Subacute Care
Require less care than acute patients but more than those with chronic → until condition is stabilized or treatment plan time is metutpO
Outpatient care
does not require overnight stay
given to those who had treatments, procedures or surgeries
Rehabilitation
help restore function after an illness or injury
Hospice Care
for those given 6 months or less to live

given to people aged 65 or above + people with permanent kidney failure or certain disabilities
