Comprehensive Assessment
A Head to Toe assessment that includes collecting history, review of systems, psychosocial information, functional status, and objective data.
Focused Assessment
An assessment that focuses on a specific area of concern using OLDCARTS and HOME questions pertinent to the complaint.
Screening Assessment
Random checks conducted in the absence of a complaint to determine if an undetected condition exists.
Emergent Assessment
A rapid assessment for ABC, Bleeding, Seizures, or Injury where objective data is collected while asking about subjective data.
Urgent Assessment Criteria
Clinical signs including Cyanosis or pallor, Respiratory difficulty, Unstable BP or pulse, Bleeding, Acute mental status changes, Seizures, and Chest pain.
OLDCARTS
A systematic approach used to explore the chief complaint during the collection of subjective data.
HOME
A systematic approach used to explore the history of the chief complaint during the collection of subjective data.
Review of Systems (ROS)
A component of subjective data collection that follows a head to toe format to identify problems in areas other than the chief complaint.
Primary Prevention
A teaching opportunity focused on the prevention of disease through immunizations and healthy habits.
Secondary Prevention
A teaching opportunity focused on identifying problems early, such as screening for HTN or Mammography.
Tertiary Prevention
A teaching opportunity focused on reducing complications with a chronic illness, such as foot care for diabetics or breathing techniques for those with COPD.
Vital Signs
Objective data collected that includes BP, Pulse, Resp Rate, Temp, O2 Saturation, and Pain rating.
General Survey
The first step in collecting objective data during a head-to-toe physical assessment.

Functional Health Status
