Describe each component of the fluency assessment
Background Information and Presenting Complaint; Case History, Parent-Child Interaction, Interview, Observation and Test Results, Summary and Prognosis, Recommendations
Case History
Demographic information, Parent/caregiver/client concerns/perceptions of the problem, problem Onset/development problem/Client's medical, family, school history/ Details about stuttering behaviors
Parent-Child Interaction
Provides more natural sample/Opportunity to see child's stuttering firsthand/Observe how parent+child interact/May be able to obtain larger, more representative sample
Interview
Opening phase, Content phase, Closing phase
Opening phase
approximately 2-4 minutes/Introduce yourself, explain purpose of visit + time frame, what's next
Content phase
"Body" or "Meat" of interview; Gather info, review case history, ask specific questions regarding dysfluencies kinds client is demonstrating, escape/avoidance behaviors, emotions towards stuttering
Closing phase
Summarize 3-5 major points (accept interruptions)/Tell them what's next/Ask if they have any questions/Thank you
Observation and Test Results
Hearing Screening/Orofacial Examination/Inclusive assessment practices - ABC's of stuttering/Stimulability
Summary and Prognosis
Differential Diagnosis
Recommendations
Post-assessment Counseling,Assessment
Affective
feelings like shame, fear
Behavioral
The specific speech behaviors exhibited prior to, during, and after a moment of stuttering
Cognitive Aspects: (thoughts, beliefs, and anticipations about stuttering, like avoiding situations or negative self-talk)
Obtaining Speech sample
size/number of samples depend on assessment purpose. first assessment, best have two samples: one clinic one client's typical environment. (preschool;homevideo),(school-age;school/home),(adult:therapy room/home/work)
Obtaining Reading sample
ensure the passage is at or below client's reading level. client's stuttering is likely to worsen when reading difficult passage; false impression of typical stuttering during reading
Assess the patterns of disfluencies
Frequency of disfluencies, Types of disfluencies
Understand the guidelines in measuring dysfluencies
multiple repetitions, single syllable word repetitions, multiple units of a phrase interjection, disfluencies separately when compounds, pauses, articulatory posture, duration, interjection, words preceding and following revisions
Record multiple repetitions of any kind as 1
"t-t-t-t-time" contains 4 repetitions of /t/, but counted as 1 part word repetition or "I was-I was going to say" contains 2 units of phrase repetition, but counted as 1 phrase repetition
Record single syllable word repetitions as 1
"we-we-we are going" contains 3 units of a single syllable word repetition, but counted as 1 word repetition
Record multiple units of a phrase interjection as 1
I had a you know-you know-you know- problem this morning contains 3 units of phrase interjection, but counted as 1 phrase interjection
Measure dysfluencies separately when they are compounded or chained
I want to -um want to-want to-to say" is a mixture of dysfluencies (2-unit phrase repetitions, an interjection, + a 2-unit word repetition), counts as 1 phrase repetition, 1 interjection, + one word repetition
Count all pauses that strike you as an undesirable duration of silence during speech is
a guideline in measuring disfluencies
If two audible disfluencies are separated by a pause, count all of them
"um (pause) um" contains two interjections + 1 pause
If there is no audible disfluency but there is a visible articulatory posture
usually involving some struggled or tensed oral movement, then count it as a silent prolongation
Record all sound prolongations as long as they are noticeable to you
a guideline in measuring disfluencies
Any pause or prolongation that is greater than 1 second is considered
abnormal
Measure the duration of 15 longest and 15 shortest disfluencies to
give a range of durations
Total number of words in sample must
also be calculated
Word count is based on
number of words or syllables that would have been spoken if person had been fluent
Interjection are counted as a disfluency, but
NOT added to the total word count, "The ba-ba-ba-baby is uh crying" contains 4 total words and 2 disfluencies
Words preceding and following revisions are counted
"The infant - the baby - is - has been crying" contains 8 total words and 2 revisions
