Myocardial infarction, coronary revasculation, or SCD before 55 yr in father or male first degree relative

Current cigarette smoker or those who quit in previous 6 months
Exposure to environmental tobacco smoke
Physical Inactivity
Not participating in ≥ 30 minutes of moderate intensity physical activity on at least 3 days per week for at least 3 months
Obesity
BMI ≥ 30 kg∙m-1 or
Waist girth > 102 cm (40 in) for men
Waist girth > 88 cm (35in) for women
Hypertension
SBP≥ 130 mmHg and/or DBP ≥ 80 mmHg
On antihypertensive medications
Dyslipidemia
LDL cholesterol ≥130mg∙dL-1 OR
HDL cholesterol <40mg∙dL-1 in men OR <50mg∙dL-1 in women OR
On lipid-lowering meds OR
Total cholesterol ≥200 mg∙dL-1
Blood Glucose
Fasting glucose ≥100 mg∙dL-1
2 hour glucose values in oral glucose tolerance test ≥140mg∙dL-1
HbA1c ≥5.7%
Elevated HDL-C (negative RF)
HDL ≥ 60 mg∙dL-1
