They sign the form online through their application.

How do students above the age of 18 submit their immunization form?

Measles, mumps, rubella, and varicella.

What are all of the immunizations that students are required to have?

Prior or current military service and/or graduation from a Tennessee high school after 2014.

What is considered proof of immunization?

Students who are enrolling in a course of study that is completely online.

What group of people are not required to have any sort of proof for immunization?

False - but they could be subject to immunization/quarantine if an outbreak were to ocurr.

True or False: students who do not provide documentation for immunization will be prohibited from attending the college.

Teacher education, early childhood education, and any programs that require child interactions, as well as nursing, lab tech, and any other allied health profession.

What programs of study require proof of compliance with immunization issued by the CDC-ACIP?

False; some specified programs, like nursing, have their own admission requirements.

True or False: admission into the college also means admission into any of our programs.

How do students switch their starting term at Motlow?

No, but test scores are used for placement purposes (i.e. learning support)
