Client Identification Protocol

Identifying the patient using at least two unique identifiers prior to procedure entry.

IV Assessment Contraindications

Conditions requiring site avoidance, including a history of mastectomy, trauma to the arm, a dialysis access site, or a previous IV location.

Bleeding Tendencies Assessment

Evaluating client risks prior to venipuncture, specifically looking for anticoagulation therapy and disease processes that impair clotting.

Preferred Venipuncture Site Selection

The most distal site in the non-dominant arm, avoiding areas distal to a previous venipuncture site, the antecubital fossa or wrist, sclerosed or hardened veins, and venous valves.

Pressure Point During Needle Removal

Apply pressure $1.5\,\text{in}$ proximal to the insertion site, away from the potential dressing site.

Catheter Flush Volume and Solution

A $10\,\text{mL}$ syringe filled with $0.9\%$ sodium chloride solution, administered while stabilizing the catheter at the hub.

Unsuccessful Insertion Protocol

Use a new intravenous catheter for each insertion attempt and do not exceed $2$ attempts before asking a coworker to attempt.
