CNA Transfer from Bed to Wheelchair

Materials

Gait Belt, Wheelchair, and Shoes

Initial steps

Place wheelchair on unaffected side against side of bed slightly angled with wheels locked and foot rests out of the way

Assist resident to sit on edge of the bed by placing hand slightly under shoulders and leaning over to place other hand under knees

Let resident sit for 10-15 seconds and check for dizziness

Assist resident to put on shoes or non-skid socks with feet flat on the floor

Stand in front of resident and apply gait belt

Grab gait belt on side and back off the resident

Ask resident to place hands on upper arms

Assist resident to stand on count of three by placing foot over their feet and other foot on the side of them

Allow resident to gain balance and check for dizziness

Move feet shoulder width apart and pivot resident

Lower them into wheelchair by bending knees and leaning forward

Align resident’s body and position foot rest then remove gait belt
