Command Officer’s Log
A daily report prepared by the on-duty Career Major that summarizes qualifying incidents and is emailed to senior staff by 0730 hours.
Operational Report
A detailed, educational report completed by the Incident Commander within 10 calendar days for significant or complex incidents that meet specified criteria.
IDLH
A fire environment that is Immediately Dangerous to Life or Health and triggers inclusion in the Command Officer’s Log when a structure fire involves it.
1-1/2” Handline
A standard firefighting hose line; its use to extinguish an IDLH structure fire must be noted in the Command Officer’s Log.
Mass or Multi-Casualty Incident (MCI)
An event with numerous victims that strains resources; must be recorded in both the Command Officer’s Log and, if eight or more victims, critiqued within three weeks.
Specialty Team Operation
Activation of units such as Haz-Mat, TRT, or Water Rescue; inclusion triggers both Log and Operational Report requirements.
Vehicle Collision with Civilian Death
Any traffic incident causing a civilian fatality; automatically recorded in the Command Officer’s Log and may require an Operational Report.
Fire/EMS Emergency Vehicle Collision
An accident involving department apparatus; recorded in the Log and, if injuries or deaths occur, requires an Operational Report.
Second Alarm (or Greater) Fire
An incident requiring resources beyond the first alarm plus task force; mandates an Operational Report and a formal critique within three weeks.
Fire Loss ≥ $50,000
Property damage threshold that triggers the need for an Operational Report.
Rapid Intervention Crew (RIC) Activation
Deployment of a standby rescue team for distressed firefighters; its activation requires an Operational Report.
Prolonged Extrication
Vehicle rescue lasting more than 20 minutes or requiring a second rescue squad; necessitates an Operational Report.
Career Major
The duty officer responsible for compiling and distributing the Command Officer’s Log each day.
Battalion Chief
On-scene or shift commander who forwards incident information to the Career Major for inclusion in the daily Log.
Incident Commander
Officer in charge of an incident; must complete and submit an Operational Report for qualifying events.
Incident Summary
Opening section of an Operational Report providing a concise overview of the event.
Incident Management Structure
Organizational chart or description of command roles included within an Operational Report.
Occupancy/Construction
Building use and structural details documented in an Operational Report to aid post-incident analysis.
