Advance Beneficiary Notice (ABN)

A document that informs covered patients that Medicare may not cover a certain service and the patient will be responsible for the bill.

Coinsurance

The agree-upon amount paid to provider by a policyholder.

Copayments

The part of an insured service the patient must pay.

Deductible

A specified amount paid by policyholder before carrier begins paying.

Explanation of Benefits (EOB)

Statement that accompanies a payment from an insurance carrier and outlines which dates and services are being paid.

Health Maintenance Organization (HMO)

Organization that provides different services through a contract with a specific group at a predetermined payment.

Preferred Provider Organization (PPO)

organization that has contract w/ providers and lease this network of contracted providers to healthcare plans.
