When looking for a ventricular rhythm what are the key identifiers?
Regular (except vfib and early/late beats), no p-waves, wide and often bizarre qrs
What to look for in a junctional rhythm
Regular (unless an early/late beat), p-wave is inverted, hidden, or follows the QRS. Narrow QRS.
What to look for in a heart block
regular, narrow qrs (unless CHB with VEF)
bundle branches are identified by
Rabbit and carrot in v1 (often also in v5), if v1 is positive RBB, v1 is negative LBB)
How to identify atrial rhythms
Irregular (except SVT), P-wave is notched, flattened, or biphasic.
Flutter vs fib
Flutter is sawtooth and waves WILL be consistent, even if its not always the same ratio, fibrillation has a more chaotic waveform
