statin indications
hyperlipidemia, primary/secondary prevention of atherosclerotic cardiovascular disease, familial hypercholesterolemia
statin mechanism
competitively inhibit conversion of HMG-CoA to mevalonate, an early rate limiting step in cholesterol synthesis. A compensatory increase in LDL receptor, which bind and remove circulating LDL-cholesterol
statin contraindications
breastfeeding and active liver disease, no bbw
statin ADR
c- myopathy, diarrhea
r- rhabdomyolysis, hepatotoxicity
statin counsel
this is used to lower bad cholesterol and raise good cholesterol, contact your doctor if you develop muscle pain
cholesterol absorption inhibitor indication
hyperlipidemia, familial hypercholesterolemia
cholesterol absorption inhibitor mechanism
inhibits the absorption of cholesterol at the brush border of the small intestine
cholesterol absorption inhibitor contraindications
gallbladder disease and severe hepatic dysfunction, no bbw
cholesterol absorption inhibitor ADR
c-none
r- rhabdomyolysis, hepatotoxicity
cholesterol absorption inhibitor counsel
this is used to lower bad cholesterol and raise good cholesterol, contact your doctor if you develop muscle pain
bile acid sequestrant indication
hyperlipidemia
bile acid sequestrant mechanism
binds with acids in the intestine to form an insoluble complex that is eliminated in the feces
bile acid sequestrant contraindications
history of bowel obstruction, triglycerides>500mg/dL, hypertriglyceridemia induced pancreatitis, no bbw
bile acid sequestrant ADR
c- constipation
r- pancreatitis, bowel obstruction
bile acid sequestrant counsel
this medication is used to lower cholesterol levels, take with food, packet/granules- mix with 120-240mL of water, stir well and drink, tablets-swallow with liquid
fibric acid indication
hyperlipidemia, hypertriglyceridemia
fibric acid mechanism
activate peroxisome proliferator-activated receptor alpha (PPARa) which increases lipolysis and elimination of triglyceride-rich particles from plasma
fibric acid contraindications
gallbladder disease, no bbw
fibric acid ADRs
c-increased transaminases
r- hepatotoxicity, rhabdomyolysis (especially in combination with a statin)
fibric acid counsel
this is used to lower LDL and TG and raise good cholesterol, this medication comes in many formulations, some require administration with food (fenoglide), contact your doctor if you develop muscle pain
antilipemic indication
hyperlipidemia
antilipemic mechanism
not well defined. may involve partial inhibition of release of free fatty acids from adipose tissue and increase lipoprotein lipase activity, which may increase the rate of chylomicron triglyceride removal from plasm. Niacin decreases the rate of hepatic synthesis of VLDL and LDL
antilipemic contraindications
severe hepatic dysfunction, peptic ulcer disease, no bbw
antilipemic ADRs
c-flushing, headache
r- hepatotoxicity, rhabdomyolysis
antilipemic counsel
this medication is used to lower cholesterol levels, you will start at a low dose and slowly increase over time to decrease side effects, to decrease flushing avoid alcohol and spicy foods while taking this medication
omega 3s indication
hyperlipidemia, hypertriglyceridemia
omega 3s mechanism
reduction in the hepatic production of triglyceride-rich very low density lipoprotein
omega 3s contraindication
hypersensitivity to drug, fish, or shellfish (use with caution) no bbw
omega 3s ADRs
c-diarrhea, fishy breath, burping, change in taste
r- elevated LFTs
omega 3s counsel
this is used to lower bad cholesterol, take with food
