• Patient performs and completes activity consistently

  and safely without physical or verbal assistance

What is the Level of Assistance of Independent with Equipment?

• Patient performs andc ompletes activity consistently and safely after equipment has been provided(cane, reacher, dressing stick, etc)

What is the Level of Assistance of Independent with Set Up?

• Patient performs and completes activity consistently and safely once the activity has been arranged, with or without the use of equipment.

What is the Level of Assistance of Supervision?

• Patient physically performs and completes the activity but requires verbal cueing and/or intermittent observation for occasional problems or inconsistent performance.

What is the Level of Assistance of Close Supervision?

• Patient physically performs and completes the activity but requires verbal cueing and continual observation.

What is the Level of Assistance of Contact Guard?

• Patient completes activity but requires therapist’s hands on for safety, not physical assistance
