Outcome Identification

SMART Goals
A framework for setting effective goals which stand for Specific, Measurable, Attainable, Realistic, and Time-bound.
Initial Planning
The phase of comprehensive planning performed with the admission nursing history and physical assessment to address each identified problem.
Ongoing Planning
Planning carried out by any nurse who interacts with the client to keep the care plan updated.
Discharge Planning
The process of planning for a patient’s care after they leave the healthcare setting, typically involving coordination with social workers and community resources.
Measurable Outcomes
Clearly defined results expected from nursing interventions that can be quantified to assess progress, such as specific pain levels or mobility goals.
Nursing Interventions
Actions taken by nurses that are valued by the client, appropriate to the client's needs, and consistent with research and standards of care.
Avoiding Errors in Writing Outcomes
Strategies to ensure clarity and measurability in client outcomes, such as using observable behaviors and avoiding general verbs.
