Confirm the patient’s identity using at least two identifiers ( e.g., name and DOB)

Verify the correct dosage , including any necessary calculations

Confirm the appropriate method of administration (oral, IV, subcutaneous)

Right Time-

Administer the medication at the correct time and frequency. Routine meds +-1 hour; STAT: within 15 mins; Procedural: exact time

Right Documentation-

Accurately document the medication given, including time, dose, and any observations

Right Reason-

Understand and confirm the rationale for administering the medication, ensuring it aligns with the patients diagnosis and condition

Common Medication Errors

• omission or wrong dose

• incorrect route or time

• failure to check allergies

• miscommunication (especially during handoffs)
