COVID-19 (mRNA or protein-based)
One dose unless > 65 y/o, which require 2 doses.
Age 2-18 years receive based on risk assessment.
DTaP for less than 7 y/o
5 doses at age 2, 4, 6, 15-18 months, and 4-6 years
Td or Tdap
Booster at age 11-12 years and then, every 10 years.
Other:
3rd trimester of each pregnancy and for close contacts of infants
wound prophylaxis - if deep/dirty wound and > 5 years since last dose (*TIG may be required).
Hib
3-4 doses at ages 2-15 months
Other: Adults with asplenia.
Hepatitis A
2 doses (6 months apart) at age 12-23 months
Other: men who have sex with men, illicit drug use, chronic liver disease, homelessness, HIV infection, and travelers to an endemic area.
Hepatitis B
3 doses at age 0 (within 24 hours after birth), 1-2, and 6-18 months.
If not previously vaccinated: age 19-59 yrs or > 60 yrs with risk factors
Hepatitis A and B
If needed prior to travel to high-risk areas.
3-dose series at 0, 1, and 6 months
HPV
Prevents cervical cancers and genital warts.
Recommended to start at age 9-12 years.
2 doses at months 0 and 6-12 if started before age of 15
3 doses at months 0, 1-2, and 6 if started after age of 15.
MMR
2 doses at age 12-15 months and 4-6 years

Pneumococcal vaccines
4 doses of PCV15 or PCV20 at ages 2, 4, 6, and 12-25 months
Adults > 50 w/ no prior vaccination or 19-49 yrs w/ certain med conditions (AUD, smoking, chronic heart/lung/liver disease, diabetes, immunocompromised state):
PCV20, PCV21, or PCV15* ×1
*Must be followed by PPSV23 ×1 > 12 months laters (or > 8 wks if immunocompromised)
IPV:IPOL
4 doses at ages 2, 4, 6-18 months, and 4-6 years
Rotavirus vaccines
Rotarix: 2 doses at ages 2 and 4 months
RotaTeq: 3 doses at ages 2, 4, and 6 months
RSV vaccines and antibodies
RSV vaccine
pregnant people at 32-36 wks of gestation during RSV season (Sep-Jan): 1 dose of Abrysvo
adults > 75 yrs or 54-74 yrs at increased risk (COPD, asthma, HF, diabetes, LTCF, etc): 1 lifetime dose
RSV monoclonal antibody
infants age < 8 month in their 1st RSV season: 1 dose if mother not vaccinated during pregnancy or vaccinated < 14 days prior to delivery.
Varicella vaccine
2 doses at ages 12-15 months and 4-6 years
adolescents/adults without immunity: 2 doses
*Stop antivirals 24 hrs before admin and don’t take for 14 days after.
*CI if allergy to gelatin or neomycin.
Zoster vaccine
2 doses at months 0 and 2-6 for adults age > 50 or > 19 yrs who are or will be immunocompromised
*2nd dose at month 1-2 if immunocompromised
*Vaccinate even if previously received Varivax or Zostavax, or has hx of zoster infection
