ICF Framework
International Classification of Functioning, Disability and Health; model that links health condition, body functions & structures, activities, participation, environmental and personal factors.
Activity Limitation
Difficulty executing a specific task or action (e.g., sitting >10 min).
Participation Restriction
Problem experienced in life situations or societal roles (e.g., inability to work).
Personal Factors
Individual background variables such as age, gender, culture, family roles influencing health.
Environmental Factors
Physical, social and attitudinal surroundings that affect functioning (home, work, climate).
Biopsychosocial Model
Holistic view that integrates biological, psychological and social influences on health.
SOAP Notes
Documentation structure: Subjective, Objective, Assessment, Plan.
Subjective Assessment
Information obtained from patient interview, case notes, reports and test results.
Objective Assessment
Physiotherapist’s observable or measurable examination, including physical tests and treatments performed in-session.
Assessment (SOAP)
Clinician’s interpretation or diagnosis derived from subjective and objective findings.
Plan (SOAP)
Proposed management: home advice, referrals, future treatments and follow-up assessments.
Orientation Assessment
Check of person, place and time to gauge cognitive status in hospital/rehab settings.
Goal Setting
Collaborative identification of desired outcomes guiding treatment and discharge planning.
Family History
Information about relatives’ medical conditions influencing genetic risk and patient perceptions.
