First Priority After an Incident
Immediate safety, which includes addressing urgent risk, obtaining medical or emergency assistance if required, and then following Lumen's incident process.
Reportable Incident Categories
Death; serious injury; abuse or neglect; unlawful sexual or physical contact or assault; sexual misconduct including grooming; and unauthorised restrictive practice.
Basic Incident Response Sequence
Safety -> emergency/medical response if needed -> document/classify -> notify where required -> investigate -> corrective action and review.
Regulated Restrictive Practices
The five regulated practices comprising seclusion, chemical restraint, mechanical restraint, physical restraint, and environmental restraint.
Unauthorised Restrictive Practice
A restrictive practice that is not in the Behaviour Support Plan, lacks required state or territory authorisation, or is used outside the conditions of the plan or authorisation.
Goal of Restrictive Practices
To use the least restrictive option for the shortest possible time, with safeguards and an active strategy to reduce and eliminate its use.
Functional Behaviour Assessment
A process used to determine the function of behaviour by gathering information from multiple sources, reviewing history and incidents, observing, collecting antecedent-behaviour-consequence data, identifying patterns, and developing and testing a functional hypothesis.
Clinical Supervision
A process providing clinical oversight, reflection, and guidance on assessment, formulation, planning, restrictive practices, and complex issues, while the practitioner remains professionally accountable for their work.
Informed Consent
Consent given voluntarily where the participant understands what they are agreeing to, including relevant choices and consequences, supported by adapted communication and decision-making where appropriate.
Email Breach Response Sequence
Contain the breach -> request deletion -> document and assess what was disclosed and the risk of harm -> notify the participant or regulator where required -> review and prevent recurrence.
Risk Register
A tool used by Lumen to identify risks, assess likelihood and consequence, document controls and further actions, and review risks through audits, governance review, incidents, complaints, and operational changes.
Continuous Improvement Cycle
Identify -> record -> assign/action -> implement -> review outcome -> close (or keep open/create further action if the problem is not solved).
Controlled Document Register
A register that identifies controlled documents and their version/review information so that the current approved version can be managed.
Risk-Assessed Role Prerequisites
Requirements before working in a risk-assessed role, including verified identity, required NDIS Worker Screening clearance, Working With Children requirements (where applicable), qualifications/competency, Code of Conduct/induction, and role-specific requirements.
Staff Training Plan
Lumen's record used to log completed training identified through supervision, self-reflection, audits, regulatory changes, incidents, and practice gaps, alongside supporting evidence.
