Healthy People 2030
A comprehensive, evidence-based national 10-year plan focused on health promotion and disease prevention to help all people achieve their full potential for health and well-being across the life span.
Community
A group of people that share a physical environment and services.
Healthy Community
A community that embraces the belief that health is more than merely the absence of disease, containing elements that enable people to maintain high quality of life and productivity.
Continuum of Care
A concept in nursing and health care aimed at providing an integrated system of care over time to decrease fragmented care and lower costs.
Continuity of Care
The degree to which a series of discrete healthcare events is experienced as coherent, connected, and consistent with the patient's medical needs and personal context over time.
Core Functions of Public Health
The three main foundational functions required to address community problems: Assessment, Policy development, and Assurance.
Family
Two or more individuals who depend on one another for emotional, physical, and/or financial support.
Caregiver
Someone who provides care to another person as a nonprofessional healthcare provider, such as parents, children, neighbors, or home health aides.
Secondary Caregivers
Nonprofessional caregivers who assist with intermittent chores such as shopping or transportation.
Population
A collection of people who share one or more personal or environmental characteristics.
Safety Net
Healthcare services designed to cover patients who are not able to pay for care themselves.
Comprehensive Services
An approach to healthcare delivery best described as 'one-stop shopping' that combines multiple health services in a single setting.
Health Disparity
An inequality or gap in health status or healthcare access that exists between two or more groups.
Chronic Disease
A long-term condition often connected to unhealthier lifestyle behaviors that represents a major factor affecting health promotion and disease prevention.
Health Promotion
Interventions focused on changing lifestyles to maximize health, serving as an important component of primary prevention.
Social Determinants of Health
Conditions affecting an individual's health status that the World Health Organization (WHO) identifies as more important to health than health care or individual lifestyle choices.
Disease Prevention
Interventions concerned with stopping the development of disease as well as treatment to prevent progression that can lead to complications.
Vulnerable Populations
A group of people at increased risk of developing health problems.
Health (WHO Perspective)
A state of complete well-being rather than merely the absence of illness or disease.
Medical Home Model
A multidimensional solution offering planned, clinically integrated care designed to meet the ongoing needs of patients with chronic disease.
Self-Management
A care model focused on patients and their active role in maintaining their own health using provided healthcare resources.
Hospice Care
Care of the terminally ill that supports quality of life for as long as possible, typically for patients who have 6 months or less to live.
Palliative Care
Care directed at alleviating symptoms and meeting the physical, emotional, and practical needs of both the patient and their family.
Case Management
A collaborative approach utilizing coordination and communication to avoid fragmented and unnecessary care that may be costly.
Complementary and Alternative Medicine (CAM)
A group of medical and healthcare practices and products that are not presently considered part of conventional medicine.
National Center for Complementary and Alternative Medicine (NCCAM)
An entity within the National Institutes of Health (NIH) that researches and evaluates non-conventional medical practices.
Epidemic
The rapid spread of an infectious disease within a community or localized region.
Pandemic
The global spread of an infectious disease across multiple countries or continents.
