Capitation
A fixed payment per patient paid to a provider or plan, regardless of the services used.
Fee-for-service
A payment model where providers are paid for each individual service delivered.
Health Maintenance Organization (HMO)
A managed care organization that requires using in-network doctors and referrals for specialty care.
Managed care
A system that coordinates and controls costs of healthcare services.
Patient-centered medical home (PCMH)
A team-based primary care model focused on comprehensive, continuous care.
Point of service (POS)
A hybrid plan combining HMO and PPO features; offers flexibility but can be more costly.
Value-based plan
A plan that pays for quality and outcomes rather than quantity of services.
