Rapid Acting Insulin
Lispro (humalog)
Aspart (Novolog)
Glusline (Apidra)
Onset: 10-30 mins
Peak: 30 mins - 3 hours
Duration 3 to 5 hours.
Give 30 mins before meals: 3x day w/ meals
short acting insulin
Regular Insulin:
Humulin R
Novolin R
Onset 30 to 60 minutes.
Peak 2 to 5 hours.
Duration 5 to 8 hours.
Give 30 to 60 minutes before a meal.
Only insulin may be IV.
intermediate acting
NPH
Humulin H
Novlin R
Onset 1.5 to 4 hours. Peak 4 to 12 hours. Duration 12 to 18 hours.
Can be mixed with rapid- or short-acting insulin.
Long Acting Insulin
Glargine (Lantus)
Detemir ( Levamir)
Degludec (tresiba)
Onset 1 to 2 hours.
No peak.
Duration about 16-24 hours.
Do not mix with other insulin.
Metformin Glucophage
First line type 2 diabetes medication. Take with meals. Watch for lactic acidosis. Hold around IV contrast as instructed.
Levothyroxine Synthroid Levoxyl Levothroid
Thyroid replacement for hypothyroidism. Take in morning before breakfast on an empty stomach with a full glass of water.
Propylthiouracil PTU
Antithyroid medication for hyperthyroidism. Decreases thyroid hormone formation. Monitor thyroid labs.
Alendronate Fosamax
Slows bone breakdown. Take with plain water and remain upright. Watch for GI and esophageal irritation.
