General ROS

Fever, chills, fatigue, malaise, generalized weakness, appetite changes, weight changes

Integument ROS

hair or nail changes , rash, itching, dryness, sores, lumps, pigmentation

Head ROS

Headache, head injury, dizzy or lightheaded

Eyes ROS

change in vision, blurry vision, diplopia, spots, flashing lights, excessive tearing, discharge, pain, redness, dryness, itching, prescription

Ears ROS

hearing loss, pain, discharge, tinnitus, vertigo, aid

Nose ROS

nasal congestion, discharge, itching, epistaxis, sinus problems

Mouth ROS

teeth and gum condition, bleeding gums, dentures, oral sores/ulcers, soreness of tongue, dry mouth, halitosis

throat ROS

soreness, hoarseness, dysphagia (trouble swallowing)

Neck/Lymphatics ROS

lumps, "swollen glands" (lymphadenopathy), goiter, neck pain or stiffness

Breasts ROS

lumps, soreness, discharge, skin changes

Respiratory ROS

dyspnea (SOB), wheezing, cough, sputum (color and quantitiy), hemoptysis, TB

Cardiovascular ROS

Chest pain, SOB dyspnea on exertion, paroxysmal nocturnal dyspnea, orthopnea (breathing on back), palpitations, edema, murmur, arrythmias, claudication, varicose veins

Gastrointestinal ROS

nausea, vomitting, diarhhea, constipation;

abdominal pain, heartburn, indigestion, trouble swallowing, painful swallowing;

rectal bleeding, black/tarry stools, vomitting blood;

food intolerance, excessive belching or flatus;

bowel habit change, hemorrhoids;

juandice, clay colored stool

Urinary ROS (MALE)

hesitancy, dribbling, decreased stream force

Urinary ROS male and female

dysuria, frequency, urgency, hematuria, nocturia, incontinence, h/o stone, flank pain

Genital ROS male

testicular pain or mass, penile lesions, urethral discharge, hernia, ED

Genital ROS female

Menstrual regularity, frequency, duration, amount of bleeding.

Dysemorhhea, PMS, menopausal symptoms, post-menopausal bleeding, vaginal discharge, itching, odor, sores, lumps, dyspareunia, post coital bleeding, spotting, sex drive. LNMP

Musculoskeletal ROS

joint pain, swollen or stiff joints, back pain

Neurological ROS

fainting, blackouts, seizures;

focal weakness or paralysis;

numbness or loss of sensation, altered sensation;

tremors or involuntary movements, imbalance

memory

Hematologic ROS

Easy bruising or bleeding, anemia/blood abnormality

immunologic ROS

reccurent or frequent infections

Endocrine ROS

heat or cold intolerance, excessive sweating, h/o goitor or thyroid trouble, polyuria, polydipsia, polyphagia

Psychiatric ROS

Excessive nervousness, tension, worry:

depressed mood;

sleep disturbance;

substance use

family/relationship problems
