functional nursing

focuses on task and jobs

each nursing team is assigned certain tasks and jobs

team nursing

involves a team of nursing staff led by an RN

(RN LPNS LVNS, and nursing assistants)

team leader (RN) delegates care to other nurses and assistants

everyone works as a team

primary nursing

the primary nurse(The RN) is responsible for persons total care

nursing team assists if needed

RN makes discharge plans and talks to family

case managers

coordinate admission to discharge and into the home or long term care setting

coordinates care

patient focused care

service moved from department to bedside

nursing team performs basic skills usually done by other health team members

LPN/LVN

supervised by RN and doctors

fewer responsibilities less supervision when person care is simple

assist RN with complex procedures

Director of nursing

responsible for entire nursing staff and care given

always and only an RN

medicare

federal program for persons 65 years of age or older

some younger with disabilities qualify

Medicaid

jointly funded by federal government and the states

for people with low income

also children and older blind and disabled people

chain of command

The don is in charge of the entire nursing staff

the nurse supervisor or manager is in charge of a nursing unit or work shift’

the charge nurse oversees RNs and LPNs for a certain shift

hospitals

provide emergency care surgery nursing care x-ray procedures and treatments and laboratory testing

is an acute care agency

persons cared for in hospitals are called patients

either in patient or out patient

out patient is when the person does not stay overnight

admission is the official entry

hospice

health care agency or program that promotes comfort and quality for dying person and the family

can be at home nursing home hospital given at home stand alone unit

no longer tries to create cures and treatments

usually less then 6 months to live

ALR assisted living residences

provides housing personal care support services health care and social activities in a homelike setting to people who need help with daily activities

are part of nursing centers or retirement communities

doesn’t receive complex medical care

regulations

made by government agencies

licensure- a state license is required to operate and provide care

certification- this is required to receive Medicare and Medicaid funds

accrediting- have standards accreditation is voluntary

long term care vs home care

long term care is for persons who cant care for themselves but do not need hospital care some need care till death, often discharged from illness or surgery offers complex care

home care provides services to persons living at home health teaching nursing care physical therapy and rehabilitation services

memory care

memory care unit is designed for persons with alzheimer’s disease and dementia

unit is closed off from other parts of center provides a safe setting where residents can wander freely

subacute and rebalitation

offers complex care medical care or rehabilitation when hospital care is no longer given

surveyor

a survey is a formal review of an agency

a surveyor is a person who collects information by observing

they will observe how care is given check for safety and cleanness

checks if standards are met

during process your job is to protect the residents rights

privacy act

only people that should have access to someone’s medical records are staff members involved in persons care

patients chart is a legal document and a permanent record

never erase or white out a mistake in a persons chart

never use pencil in a persons chart

nursing assistants document on flowsheets

CNA in nursing process

nursing assistants have a key role by observing reporting and recording

Observe and report changes in the resident's condition to the nurse. The CNA does not assess the patient or decide what should be done but your observations do help
