Bisphosphonates-Alen “dronate” (Fosamax)
Mode of action
Decrease osteoclastic activity, which decreases bone resorption for osteoporosis
Side effects
Esophageal ulceration
Headache, nausea, diarrhea, dyspepsia
Clinical Pearls
Taken with full glass of water and must remain upright for 30 min
Taken on an empty stomach
Separate iron, antacids, and vitamins for 30 minutes
Encourage increased calcium and vitamin D
Allopurinol (Aloprim)
Uses- to treat Gout
Therapeutic class: uric acid inhibitor
MOA:
reduces uric acid production
Adverse reactions
N/V/D, renal failure, rash
Monitoring:
✓ uric acid levels, CBC, LFT’s and renal function
Does not help with acute attacks
Colchicine
Relieves acute gout attacks + prevents
Does not help with pain relief
Side effects
Nausea, vomiting, abdominal pain, diarrhea
**bone marrow suppression
Gout Treatment
NSAIDS or steroids
Colchicine (Colcrys)-Antigout agent
Begin with symptom onset
Rapid symptom relief, within 12hrs
Side effects- diarrhea
Allopurinol (Zyloprim)-Uric Acid Inhibitor
blocks uric acid production
Side effect- rash or Stevens-Johnson syndrome
