I took an internship at Muscle Shoals Municipal Hospital in Alabama. Thereafter, I completed my residency at the Deadman Medical Center in Dallas. I was then hired as a general practitioner at a small medical practice

I treated walk-in patients who had a variety of different illnesses. Sort of a jack-of-all-trades

Well funny enough I ran into Nano at our highschool reunion, and then he offered me a job as his personal physician.

  1. What did being Mr. D’Sign’s personal physician entail?

I performed basic physical checkups once a week, did blood work once a month, and gave him an extensive physical and cognition test once a quarter. I was also available on call 24/7, in case an emergency arose.

No, he paid me well, so there was no need to. It worked well actually, because all my attention was focused on him and his health

Yes, I did, I was quite surprised actually, Nano has never called me personally. He stated he needed a full physical, stating that he required it for a scuba dive

Yes, Mr. Eubanks, his assistant was ALWAYS present

We both worked for Nano, and I have seen Nano and Mr. Eubanks interact many times when I would visit Nano’s residence

He would always call him Alex.

Never. In the 5 years that I worked for Mr. D’sign, he has never referred to Alex as ACE.

It entails the basics. I check his vital, his weight, height, blood pressure, and heart rate.

84%, which was lower than previous tests, but still within the normal 80-100 range.

As you can see here, I asked Nano if he was having any breathing issues, such as wheezing or coughing, to which he denied. I saw no asthmatic symptoms, so I left it at that.

No I did not, he had a full range of motion, his motor skills were functioning normally

Not at all

Yes, I would conduct a standard Mini-Mental State Examination, or MMSE, quarterly

  1. What does this MMSE entail?

It screens for signs of dementia, using a 30 point scale. Any score at or below 24 indicates dementia at varying levels.

  1. How did Mr. D’Sign perform on these exams?

He never got below a 25, meaning evidently he never showed any signs of dementia

  1. Would you ever test Mr. D’Signs handwriting on these exams?

Yes I would,  a portion of the exam included having Nano write a sentence or draw something. Nano struggled with those, but it was only because he rarely wrote by hand. And it was never enough to lower his score under 25 points

I’ve known Nano since high school; he NEVER wrote by hand. He would make such a huge fuss about it with the administration back then. Knowing him, I knew his poor handwriting was not a concern, since that was just Nano.

Yes I am, since I tested them quite frequently on Nano.

  1. Did Mr. D’Sign ever exhibit signs of dementia that you personally observed?

I have examined Nano over a 100 times, both in physical examinations and more intensive cognition exams, and in my personal observations, he has never exhibited anything that would raise concerns about his mental state.

A minor incident, yes, but nothing that raised any concern

  1. And what was this minor incident?

Well, Nano was performing a Stress ECG test to monitor his heart and blood pressure levels. He was walking on a treadmill with a bunch of wires on him, when about 10 minutes in towards the end of the test, he tripped

  1. What happened after he tripped?

He was okay, but just some minor bruises. Patients trip all the time. He was pretty embarrassed, so I decided that I had all the information I needed, and I cleared him for the scuba dive.
