Step 1
Look up the disease, injury, or condition in the Alphabetic Index in ICD-10-CM. Look up in the Tabular List to make sure the diagnosis code is correct.
Step 2
After you have the correct diagnosis code from the Tabular list, refer to the CPT book for the procedural codes. Find the part of the body that the procedure involve and go to the section of the book for that part of the body.
Step 3
Diagnosis code should support the medical necessity for the procedural code you’re submitting.
Example of Step 3
If you are reporting a hernia repair the diagnosis must be a hernia.
