Uses gravity
Moves bronchial secretions toward central airways
Secretions can then be removed by:
Cough
Mechanical aspiration
What is percussion?
Rhythmic cupping/clapping
Applied over lung segment being drained
Helps mobilize secretions
What is vibration?
Fine tremulous action over chest wall
Applied over affected lung segment
Performed during exhalation
How long should each postural drainage position be maintained?
2–3 minutes
What are major contraindications to postural drainage positions?
ICP >20 mmHg
Unstable head/neck injury
Active hemorrhage/hemodynamic instability
Active hemoptysis
Empyema
Bronchopleural fistula
CHF-associated pulmonary edema
Large pleural effusion
Pulmonary embolism
Rib fracture/flail chest
Surgical wound/healing tissue
