General Rules for Nursing Math

The essential procedural directives for nursing calculations: always read the directions, label all answers, and put the final answer on the line next to the problem.

Trailing Zeros Rule

The rule stating that trailing zeros must NOT be placed to the right of decimal points (e.g., write $5\,\text{mg}$ instead of $5.0\,\text{mg}$) to prevent medication errors such as misreading $5.0\,\text{mg}$ as $50\,\text{mg}$.

Leading Zero Rule

The rule stating that a leading zero MUST be used to the left of a decimal point when a number is less than one and not preceded by a whole number (e.g., write $0.5\,\text{mg}$ instead of $.5\,\text{mg}$) to decrease the chance of an error.

Rounding to the Nearest Tenth

The process where if the first number to the right of the decimal point (tenths position) is equal to or greater than $5$, round up and delete remaining numbers to the right; if less than $5$, round down (do not change it) and delete remaining numbers to the right.

Rounding to the Nearest Hundredth

The process where if the second number to the right of the decimal point (hundredths position) is equal to or greater than $5$, round up and delete remaining numbers to the right; if less than $5$, round down (do not change it) and delete remaining numbers to the right.

Weight in Kilograms Rounding Rule

The rule requiring that weight in kilograms is NOT rounded to the nearest tenth before performing calculations, but is instead rounded at the end of the calculation problem.

Intermediate Steps Rounding Rule

The directive prohibiting the rounding of numbers during the intermediate steps of any problem, as doing so will result in a final dose being either too high or too low.

Final Result Rounding Rule

The requirement to round the final result to the nearest number that can be accurately measured (typically specified as the nearest whole number, tenth, or hundredth depending on the problem).
