1. MMT 3/5 R quads
1. Manual muscle test 3/5 right quadriceps.
2. Pt. can stand A for 30 sec LOB
2. Patient can stand without assistance for 30 seconds without loss of balance.
3. Pt. can transfer bed → w/c mod A using SB
3. Patient can transfer from bed to wheelchair with moderate assistance using a sliding board.
4. Pt. instructed in performing R SAQ, 3 sets ×10 reps
4. Patient instructed in performing right short arc quads, 3 sets of 10 repetitions.
5. PROM R ankle DF 5°
5. Passive range of motion of right ankle dorsiflexion is 5 degrees.
6. Received Rx from MD for WBAT on L LE
6. Received order from (or prescription from) medical doctor for weight bearing as tolerated on left
lower extremity.
7. Pt. instructed to perform HEP b.i.d., 10 reps each ex
7. Patient instructed to perform home exercise program twice a day, 10 repetitions each exercise.
8. Pt. admitted to ER 10/12/15 with GCS of 4
8. Patient admitted to emergency room on 10/12/15 with a Glasgow Coma Scale score of 4.
9. Medical dx: R hip fracture ORIF
9. Medical diagnosis: right hip fracture with open reduction, internal fixation.
10. AROM B LEs WNL
10. Active range of motion in bilateral lower extremities is within normal limits.
11. Pt. instructed in use of TENS unit, on prn basis
11. Patient instructed in use of transcutaneous electrical nerve stimulator unit on as needed basis.
12. CPT ×20 min; P&V RLL
12. Chest physical therapy for 20 minutes, percussion and vibration to right lower lobe.
13. Pt. was d/c'd from NICU on 3/3/15
13. Patient was discharged from the neonatal intensive care unit on 3/3/15.
14. PMH: IDDM ×5 yr, HBP ×10 yr
14. Past medical history: insulin-dependent diabetes mellitus for 5 years, high blood pressure for 10
years.
MRI revealed mod L MCA CVA
15. Magnetic resonance imaging revealed moderate left middle cerebral artery cerebral vascular
accident.
