Intake/output sheet
Graduate (measuring container)
Urine in bedpan/urinal
Gloves
Paper towels/bed protector sheets
Alcohol wipes
What is the first step for this skill?
Don gloves
Place barrier on to floor to avoid contact between drainage bag and floor
What is the second step?
Untuck nozzle of urinary bag and drain into graduate/bedpan
Avoid contamination of the tube
What is the third step to this skill?
Wipe drain tube clean and tuck back into its original place
Lock the tube (with clamp)
What is the fourth step to this skill?
Set graduate/measuring container on flat surface and obtain measurement
Empty contents into toilet and clean/rinse equipment
Remove gloves and wash hands
What is the fifth step for this skill?
With clean hands, record measurement
Record within +/- 50 mL of nurse’s measurement
Record output as urine, record the time on I&O form
What should you make sure to do?
Check if drainage bag is hanging from bed frame and not touching the floor
Keep urinary drainage bag positioned lower than bladder throughout care
