Signs/Symptoms - Signs are objective, what’s visible. Symptoms are subjective, what the patient is feeling.

A

Allergies - Drug AND food allergies. Also ask how they react to these allergies.

M

Medications - Prescription meds? Over the counter meds? Home remedies?

Important to know if meds are being used improperly (not at all, too much, etc), and what the meds do (ex. blood thinners) since their effects can impact treatment.

P

Past History - Any pertinent medical history, such as conditions, recent surgeries/operations, or other incidents like this.

L

Last oral intake - Exactly when the patient last ate or drank something, medication included.

E

Events - A&O round 2 using AVPU.
