Patient-Centered Medical Home (PCMH)
Primary care model that coordinates patient care to ensure patients receive needed services when and where they need them.
Goal of the PCMH
Create a centralized setting that promotes partnerships among the patient, provider, and family.
Comprehensive care
Addresses all of the patient's needs (whole-person care) using the provider and entire health care team.
Patient-centered care
Places patients and their families at the center of care, focusing on individual needs and preferences.
Coordinated care
Ensures specialty care, hospitals, home health, and community services are organized by the primary care provider using communication and health IT (EMR/EHR).
Accessible services
Provides tools such as open scheduling, extended hours, provider communication, and patient portals.
Quality and safety
Delivers evidence-based care while measuring safety, patient experiences, and satisfaction to improve outcomes.
