Diphtheria, Tetanus, and Pertussis (DTaP)
5 dose series at ages 2, 4, 6, 15-18 months and 4-6 years
tetanus toxoid
protects for 10 years, given in combo when receiving as a primary immunization in infants, given alone or in combo with diphtheria for adults
herpes zoster vaccine
2 dose series given 2-6 months apart to prevent shingles in adults aged 50 years and older
influenza
recommended for infants and children 6 months+, recommended annually in Sept and Oct for maximum immunity, 0.5mL IM single dose
hepatitis A
2 dose series with 1st dose between 1-2 years of age and 2nd dose at least 6 months after; recommend all children at age 1 and workers in high risk jobs
human papillomavirus (HPV)
given to adolescents aged 11 or 12, most effective if given before the person becomes sexually active, prevents genital warts and cervical cancer risk, 3 dose series with 2nd dose 2 months after 1st and 3rd dose 6 months after 1st dose
measles, mumps, rubella (MMR)
0.5mL IM at 12-15 months and 2nd dose at 4-6 years
