signed by recipient, states name and address, occupation, specify total quantity of required, specify purpose for which CD is required 

Why may someone request CDs from a pharmacy?

Drs or dentists to keep in the surgery/clinic for stock purposes 

What should the pharmacy mark on the requisition?

name and address

Where are requisitions sent at the end of the month?

NHSBSA

How long should a copy of the requisition be kept?

2 years

When can a schedule 2/3 be supplied to a doctor with no requisition?

in an emergency, only if the requisition can be supplied in the next 24 hours

Does a pharmacy requesting CDs need to send a requisition?

its advised but not a legal requirement

Who can collect the CDs from the requisition?

the person themselves, or a messenger who has a written statement to say they are collecting drugs on the doctors behalf
