What are some other barriers besides language that could come up when using a questionnaire?

What is the procedure for administering a questionnaire?

Look at the slide (pretty basic) - mainly needed for practical purposes

Disabilities of the arm, shoulder, and hand (DASH)questionnaire

• Answers should be based on last week (or best estimate if no activity in last week)

• Can be performed with either UE

• 30 ?s with optional work and sports sections

Owestry Low Back Pain Questionnaire

• Used to determine permanent limitations in functional ability

• Considered the gold standard for low back functional outcome tools

• 10 ?s regarding various parts of life

Knee Outcome Survey (KOS)

• 14 ?s used to determine how their knee symptoms are affecting their ability to perform ADLs

• Considered to have high test-retest reliability

Foot Function Index

• Used to develop measure the impact of foot pathology on function in terms of pain, disability and activity restriction

• 23 items divided into 3 sub scales

• Pt answers questions based on their pain over the previous week

• Tests-retests reliability was studied and found to be good

Western Ontario and McMaster University Osteoarthritis Index (WOMAC)

• Used for pts with osteoarthritis in the hip and/or knee

• 24 ?s divided into 3 subsections

Fear Avoidance Beliefs Questionnaire (FABQ)

• Used to determine fear-avoidance beliefs among pts with low back pain

• A low score is linked to higher success rates with specific Rx

• Demonstrated to be valid and reliable (in pts with chronic low back pain)
