Healthy People 2030
A national initiative managed by the Office of Disease Prevention and Health Promotion (ODPHP) that sets goals and objectives to address preventable threats to health.
ODPHP
Office of Disease Prevention and Health Promotion, the organization responsible for establishing goals and objectives under Healthy People 2030.
Social Determinants of Health
The conditions in the environments where people live, work, learn, play, and worship that affect a wide range of health outcomes and risks.

Health Belief Model
A health behavior model comprising six primary components: Perceived Susceptibility, Perceived Severity, Perceived Benefits, Perceived Barriers, Cue to Action, and Self-efficacy.
Perceived Susceptibility
A construct of the Health Belief Model referring to an individual's assessment of their subjective risk of developing a health problem.
Perceived Severity
A construct of the Health Belief Model referring to an individual's assessment of the seriousness and consequences of a health condition.
Perceived Benefits
A construct of the Health Belief Model referring to an individual's assessment of the value or effectiveness of engaging in a health-promoting behavior.
Perceived Barriers
A construct of the Health Belief Model referring to an individual's evaluation of the obstacles, costs, or challenges associated with taking a health action.
Cue to Action
A construct of the Health Belief Model referring to internal or external triggers that prompt an individual to execute a health-related behavior.
Self-efficacy
A construct of the Health Belief Model defined as an individual's personal confidence in their ability to successfully perform a health action.
Stages of Change Model
A model of behavior change outlining six sequential stages: Precontemplation, Contemplation, Preparation, Action, Maintenance, and Termination.
Precontemplation
The initial stage in the Stages of Change Model where an individual has no intention to modify behavior or take action in the foreseeable future.
Contemplation
The stage in the Stages of Change Model where an individual intends to change behavior in the foreseeable future and weighs the pros and cons of action.
Preparation
The stage in the Stages of Change Model where an individual intends to take action in the immediate future and begins taking small steps toward change.
Action
The stage in the Stages of Change Model where an individual actively makes specific, observable modifications to their lifestyle or habits.
Maintenance
The stage in the Stages of Change Model where an individual works to sustain behavior changes over time and avoid returning to former habits.
Termination
The final stage in the Stages of Change Model where an individual has zero temptation to relapse and complete self-efficacy in maintaining their change.
