120 Rule in lung function assessment
A clinical guideline that states PaCO2 + PaO2 should equal greater than 120 mmHg, allowing clinicians to assess lung function quickly in room air (21% oxygen at sea level).
Hypoventilation assessment using the 120 Rule
If PaCO2 = 60 mmHg and PaO2 = 85 mmHg, then PaCO2 + PaO2 = 145 mmHg, indicating the patient is suffering from hypoventilation only.
Hypoxemia and admixture
If PaCO2 = 60 mmHg and PaO2 = 40 mmHg, PaCO2 + PaO2 = 100 mmHg, indicating the patient has both admixture and hypoventilation.
Admixture without hypoventilation
If PaCO2 = 40 mmHg and PaO2 = 60 mmHg, PaCO2 + PaO2 = 100 mmHg, indicating the patient has admixture without hypoventilation.
