Orientation, Identification, Exploitation, Resolution Phase

The four phases of a nurse client relationship

Orientation phase

It's the problem defining phase. Starts when the client meets nurse as a stranger

Orientation phase

Client seeks assistance. Nurse responds, explains roles to the client

Identification phase

Selection of appropriate professional assistance

Identification phase

Patient begins to have a feeling of belonging and a capability of dealing with the problem which decreases the feeling of helplessness and hopelessness

Identification phase

Patient identifies with those who can help him/her

Exploitation phase

Goals are implemented by the nurse but power is shifted to the patient as these goals would be achieved through personal or self-effort

Exploitation phase

Dependent person becomes independent.

Exploitation phase

Nurse aids the patient in exploiting all avenues of help and progress is made towards the final step

Resolution phase

In this phase, the client no longer needs professional services and gives up dependent behavior. The relationship ends.

Resolution phase

The patients needs have already been met by the collaborative effect of patient and nurse

Resolution phase

Now they need to terminate their therapeutic relationship and dissolve the link between them

Stranger

Should treat the patient courteously, should not prejudge
the patient. Offering the client, the same acceptance and courtesy that the nurse would to any stranger

Resource person

Provide specific needed information that aids in the
understanding of a problem or new situation. Providing specific answers to question within a larger context

Teacher

Must determine how the patient understands the subject at hand, must develop her discussion around the interest of the patient & ability of using the information
provided

Leader

Helps client assume maximum responsibility for meeting treatment goals in a mutually satisfying way,
offering direction to client or group.

Counsellor

helps to understand and integrate the meaning of current life circumstance, provides guidance and encouragement to make changes. Promote experiences leading to health for the client such as expression of feelings

Surrogate

helps to clarify domains of dependence and interdependence and independence and acts on client behalf as an advocate

Technical expert

providing physical care for the patient and operates equipment
