prox = sides of T12 and L1 vertebrae and their IV discs, distal = pectineal arch (leads to iliopubic eminence and pubis)

psoas major

prox = transverse processes of lumbar vertebrae, sides of vertebral bodies T12 - L5 and IV discs in between, distal = lesser trochanter

iliacus

prox = superior 2/3 of iliac fossa, ala of sacrum and sacroiliac ligaments, distal = lesser trochanter

pectineus

prox = superior ramus of pubis, distal = posterior superior femur (inferior to lesser trochanter)

rectus femoris

prox = AIIS, distal = quadriceps tendon, patella, patellar tendon, tibial tuberosity

vastus medialis (oblique)

prox = intertrochanteric line and medial lip of linea aspera of femur (medial aspect of the vertical line - posterior aspect of femur), distal = quadriceps tendon, patella, patellar tendon, tibial tuberosity

vastus lateralis

P - greater trochanter and lateral lip of linea aspera (vertical line on posterior aspect of femur)

D - quadriceps tendon, patella, patellar tendon, tibial tuberosity

vastus intermedius

prox = anterior and lateral surfaces of shaft of femur, distal = quadriceps tendon, patella, patellar tendon, tibial tuberosity

adductor longus

prox = pubis, distal = middle 1/3 of linea aspera of femur/posterior vertical line of femur

adductor brevis

D - proximal part of linea aspera of femur/posterior vertical line of femur

adductor magnus (adductor part)

prox = pubis and ramus of ishium, distal = linea aspera (vertical line posterior femur), medial supracondylar line

adductor magnus (hamstring part)

prox = ischial tuberosity, distal = adductor tubercle of femur

gluteus medius

prox = external surface of ilium, distal = lateral surface of greater trochanter

gluteus minimus

prox = external surface of ilium, distal = anterior surface of greater trochanter

gluteus maximus

prox = posterior ilium, dorsal sacrum and coccyx, erector spinae aponeurosis, sacrotuberous ligament, distal = iliotibial band and superior portion of posterior tibia

iliotibial band

prox = prox attachments of gluteus maximus and TFL, distal = gerdys tubercle of tibia

tensor fascia latae

prox = ASIS and anterior iliac crest, distal = iliotibial band which attaches to gerdys tubercle

piriformis

prox = anterior surface of sacrum, distal = superior greater trochanter

biceps femoris

prox (long head) = ischial tuberosity, prox (short head) = linea aspera (posterior vertical line femur) and lateral supracondylar line of femur, distal = lateral aspect of head of fibula

medial (tibial) collateral ligament MCL

prox = medial epicondyle of femur, dist = superior medial condyle tibia

lateral (fibular) collateral ligament LCL

prox = lateral epicondyle femur, dist = superior lateral fibula

anterior cruciate ligament ACL

prox = posterior part of the medial surface of the lateral femoral condyle, dist = anterior intercondylar area of tibia

posterior cruciate ligament PCL

prox = anterior part of the lateral surface of medial femoral condyle, dist = posterior intercondylar area of tibia

medial meniscus

anterior = anterior intercondylar area of tibial plateau anterior to ACL, posterior = posterior intercondylar area of tibial plateau anterior to PCL

lateral meniscus

A - intercondylar fossa on tibial plateau close to posterior attachment of lateral meniscus

P - intercondylar fossa on tibial plateau close to anterior attachment of lateral meniscus

gastrocnemius

prox (medial head) = posterior surface of femur, superior to medial condyle, prox (lateral head) = lateral condyle of femur, dist = calcaneal tendon posterior surface of calcaneus

soleus

prox = posterior aspect of head of fibula, soleal line, and middle 1/3 of medial border of tibia, dist = calcaneal tendon posterior surface of calcaneus

popliteus

P - lateral condyle of femur

D - posterior surface of tibia, superior to soleal line

tibialis posterior

prox = interosseus membrane, posterior surface of tibia and fibula, dist = navicular tuberosity and across plantar surface of foot (cuneiforms, cuboid, calcaneum) to the base of 2nd to 4th metatarsals

flexor digitorum longus

prox = medial posterior surface of tibia, dist = plantar surface of distal phelanges of lateral 4 digits

flexor hallucis longus

prox = posterior surface of fibula and inferior IOM, dist = plantar surface of distal phalanx of hallux

fibularis longus

prox = head and superior 2/3 of lateral surface of fibula, dist = plantar surface of base of 1st metatarsal and medial cuneiform

fibularis brevis

prox = inferior 2/3 of lateral surface of fibula, dist = lateral surface of base of 5th metatarsal

tibialis anterior

prox = lateral condyle and superior half of lateral surface of tibia and IOM, distal = medial and plantar surface of medial cuneiform and base of 1st metatarsal

extensor hallucis longus

prox = middle part of anterior surface of fibula and IOM, distal = dorsum of distal phalanx of hallux

extensor digitorum longus

prox = lateral condyle of tibia, superior surface of fibula and IOM, dist = dorsum of middle and distal phalanges 2-5

fibularis tertius

prox = inferior anterior surface of fibula and IOM, dist = dorsum of base of 5th metatarsal
