Logic Model

The organizing framework. Includes inputs (what you need ex. Resources, partnerships, funding, data gathering), activities, outputs (what will you accomplish), short-term outcomes (6-12 mo), and long-term outcomes (1-3 yrs)

Implementation Plan

Roadmap that includes who will do what, timelines, resources, and monitors progress

Formative Evaluation

Early implementation and planning. Assessing how feasible the program is and finding ways to improve the process

Process Evaluation

What will the program do? What activities/services can show the intervention was implemented as intended. Doesn’t say whether or not the outcome was reached

Summative/Outcome Evaluation

Did it accomplish what it intended to? How will you measure if what you did made a difference? Ex. checklists, assessments, and pre/post tests

Community

A localized group that shares identity and geography

Population

An aggregate group that shares common attributes or health outcomes

Community Health Assessment

Community core, Environment, Health/Social services, Economy, Education, Safety/Politics, Communication, and Recreation

Qualitative Data

Windshield survey objective observations, and findings from key informant interview

Quantitative Data

Census data, health indicator data, and comparing community to MD and USA
