Personal Health Information Protection Act (PHIPA)
Legislation that governs health care information privacy in Ontario.
Custodian
An organization that provides care within the health care continuum and is responsible for the management of personal health information.
Explicit Consent
Specific permission obtained from a client before disclosing personal health information.
Implied Consent
Assumed permission for sharing personal health information among health care team members to provide care.
Lockbox Provision
Client's right to instruct that a portion of their health information not be shared with other providers.
Personal Health Information
Any identifying information about clients in verbal, written, or electronic form related to their health.

Failing to protect client confidentiality and privacy as regulated by the Nursing Act.
Mixed Record
A record that contains both personal health information and other information that isn't classified as personal health information.
Knowledgeable Consent
Understanding and agreement by the client regarding the collection, use, and disclosure of their personal health information.
